The Nurse Navigator is an RN Case Manager who works within the Trinity Clinic Practice groups to assess the patient, develop a Care Plan with the patient, family and physician, facilitate care and improve overall health and wellness of patients with complicated chronic disease and multiple co-morbid conditions. The Nurse Navigator will facilitate communication with the treatment team, all departments within the Clinic and hospital facilities and promote community resources to provide medical management of this patient population and to assess ongoing health care needs to provide community resources and referrals.
* Graduate of an accredited school of professional nursing. Bachelor's of Science in Nursing (BSN) is preferred. * Must be licensed as a Registered Nurse with the State of Texas. * Prefer candidate with CCM Certification (Commission for Case Manager Certification), or must be willing to obtain within 1 year of employment. * Requires 2-3 years experience with home health case management; acute care RN and case management experience is a plus. * Must have knowledge of Managed Care, health plan design, clinical knowledge and utilization reporting and outcomes management. * Must be proficient with Microsoft Office - Word, Excel and Powerpoint. * Must have Electronic Medical Record experience. Perfer EPIC * Must have strong presentation, negotiation and conflict resolution skills, as well as excellent written and verbal communication skills.
