Seeking qualified Pharmacist to participate as part of the care team providing medication utilization review and consultation on prescribed medications, drugs, and other pharmaceuticals as they relate to patient care.
This position will work as part of the care team providing medication reconciliation with patients. Provides recommendations and guidance to medical providers on optimal pharmaceutical therapies. Oversees 340B program and manages voucher program to ensure pharmacies are not defaulting patient insurance to the voucher program. Oversees drug sample program.

Qualified candidate must be a graduate of an accredited school of pharmacy and be licensed as a pharmacist by the State of Oregon. At least one year of experience is preferred. Bilingual in English/Spanish preferred, but not required.
