Coordinates with the RN to provide skilled nursing care as approved by the patient's physician, demonstrating knowledge and compliance with all applicable Agency policies and procedures, state and federal guidelines, CHAPS standards, and parameters of licensure. Follows care pathway as outlined when providing care to terminally ill patients as established by the RN; clarifies specifics prior to visit. Assists in identifying patient/family education and care needs in order to achieve optimal patient outcomes. Identifies current and potential problems, barriers and works toward appropriate resolution. Weight:35%
Reports identified problems to the physician in a timely manner. Accepts orders and notifies the patient's case manager and supervisor as appropriate. Communicates changes in patient status, missed / moved visit activity, and schedule changes. Weight:15%
