Manager, Care Management:
Responsible for the day to day operation and performance of the care management team providing care management services to EmblemHealth members.
Monitor team member performance which includes quality, outcome and productivity measures; deploy strategies to improve performance. Provide coaching and mentoring to team members to address performance related issues. Use problem solving abilities to create solutions to operational and clinical issues identified during day to day operations.
Lead Complex Care Manager (RN)
Support EmblemHealth population health complex care management initiatives by providing care management services engaging with the most complex individuals enrolled in care management programs
Provide oversight to a multidisciplinary care team responsible for a population of health plan enrollees in a discrete “geo panel” or care pod
Engage with the most complex members of the health plan with the goal of improving health care outcomes and appropriate and timely utilization of services
Serve as the “quarterback” or “leader” of a geo pod ( care team) who will assess and triage members to the most appropriate level of support needed.
Complex Care Manager (RN)
Support EmblemHealth population health complex care management initiatives by providing care management services engaging with complex individuals enrolled in care management programs
Assess plans, implement, coordinate, monitor, and evaluate all options and services with the goal of optimizing the patients’ health status.
Integrates evidence-based clinical guidelines, preventive guidelines, and protocols, in the development on individualize care plans that are patient-centric, promoting quality and efficiency in the delivery of health care.
Coordinates care across settings and helps patients/families understand health care options

Manager, Care Management:
We require a Bachelor’s degree (preferably in Nursing or related), NY licensed RN, 5 – 8 years clinical experience, and a proven track record in leading care management team(s). Health plan experience highly desirable.
Lead Complex Care Manager (RN):
Bachelor’s degree from an accredited college or university, current New York State RN license, 5 to 8 years of clinical experience, prior health plan experience highly desirable, Certified Case Manager (CCM) required within 1 (one) year of employment.
Complex Care Manager (RN):
Bachelor’s degree from an accredited college or university, 4 to 6 years of clinical experience, current New York State RN license, and Certified Case Manager (CCM) required within 1 (one) year of employment. Prior health plan experience highly desirable.
