Reports to the Nurse Manager, Specialty Clinics. The RN chemotherapy nurses uses the nursing process and evidence-based practice to collaborate with the Core Team (patient, Oncologists and Expanded Team (family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient-driven holistic care plan for life. The RN Care Manager is a member of the core team. Demonstrates leadership and mastery of practice in delivering efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication and coordination of resources. Works in partnership with the Oncology team and panel of patients for effective oncology care management.
· Work with the Hematology/Oncology Cancer Care Case managers and the clinic clerk to provide coordination of appointments and scheduling.
· Responsible for collaborating with services internal and external to the VA to facilitate care coordination in order to effectively meet the patients' needs.
· Administering chemotherapy and blood products
· Initiating IV access
· Lab draws via port access
· Provide patient education and information to the patients receiving care and treatment.
