JOB TITLE: Infusion Coordinator Registered Nurse Case Manager LOCATION: Geisinger Wyoming Valley Medical Center DEPARTMENT: Vitaline Pharmacy WORK SCHEDULE: Days WORK TYPE: Full Time (1.0 FTE) JOB SUMMARY: Serves in an expanded nursing role with the focus on home infusion discharge planning. Acts as liaison between inpatient and outpatient providers and the infusion pharmacy clinical and support staff by collaborating with the physicians and care management staff to ensure a smooth transition for all patients and their caregivers from hospital to home. Collaborates with members of the multidisciplinary team to ensure the delivery of quality, efficient and cost effective infusion therapy in the home setting through attendance of the daily interdisciplinary team meetings at Geisinger Health System (GHS) facilities. Members of this team may include but are not limited to the attending and consulting medical staff (may include the GHS antimicrobial staff), the IV therapy team and/or Ambulatory Infusion Center (AIC), those involved with reimbursement and insurance concerns, case management staff, home health agency staff and VITALine clinical and support staff. Collaborates with discharge planning teams at non-GHS facilities, as well as home health agencies to facilitate successful discharge for clients receiving infusion services in home. Provides education to patients and their caregivers of all ages and of varied educational and socioeconomic backgrounds. Introduces them to home infusion and the role VITALine and the home health agency will play from the time of discharge through completion of the infusion therapy. Provides education at an appropriate level for patients and caregivers regarding the infusion process including but not limited to: prescribed therapy including side effects and adverse drug reactions, care of and troubleshooting for the vascular access device as well as the therapy’s method of administration. Assesses, evaluates, plans, implements and coordinates home infusion needs and required therapy services. Utilizes evidence-based clinical guidelines, protocols and procedures to develop individualized plans of care. Works closely with the VITALine Marketing Representative to increase referrals received from GHS and non-GHS providers. Assists VITALine Marketing Representative to develop and improve relationships with referral sources, including home health, assisted living facilities, skilled nursing facilities acute rehabilitation facilities. Works under the direct supervision of the Director of VITALine. COMPETENCIES AND SKILLS: Demonstrates a broad clinical knowledge base, has proficient clinical assessment skills and has a working knowledge of the care and maintenance of vascular access devices. Demonstrates knowledge of basics concepts and principles of managed care, infusion therapy and vascular access devices and their care. Demonstrates basic computer skills. Demonstrates customer focused interpersonal skills to interact in an effective manner with practitioners at all levels, the interdisciplinary health care team, community agencies, patients and their families and/or caregivers with diverse opinions, values, religious and cultural values and ideals. Demonstrates ability to work autonomously and is directly accountable for practice. Demonstrates ability to influence and negotiate individual and group decision making processes. Demonstrates the ability to function effectively in a dynamic and rapidly changing environment. Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving and decision making skills, priority setting, work delegation, and work organization skills. Demonstrates knowledge of, including but not limited to, administration of total parental nutrition, enteral nutrition, chemotherapy including handling of cytotoxic drugs and biohazardous waste, wide range of antibiotic therapy and pain management therapy. Demonstrates the ability to calculate infusion rates, appropriate dosing by weight for applicable therapies, and infusion pump program changes in response to physician orders or therapy changes. Must exhibit an understanding and a strong commitment to working in a team environment. EDUCATION AND/OR EXPERIENCE: Must be a graduate of an accredited school of nursing. Current license to practice in the state of Pennsylvania is required. Bachelor’s degree in the health care field preferred. Minimum of 3 years nursing experience with a minimum of 1 year in an acute care setting is required. Home health experience is preferred. Case management and patient education experience preferred. Valid Pennsylvania driver’s license and willingness to do work related travel as required. WORKING CONDITIONS/PHYSICAL DEMANDS: Typically works in a clinical setting/environment. May have exposure to hazardous materials and to disease and illness including but not limited to hepatitis B and HIV. Frequently stands, walks, sits and climbs stairs. Cooperates and shares responsibilities for covering week day and week end on-call services with other VITALine nursing staff to ensure 24 hour 7 day a week nursing availability.
