The role you’ll contribute:
The Patient Care Advocate position is an outreach position for AdventHealth Medical Group to assess patients who need care for a variety of medical conditions that are identified by data analysis. Chart reviews will be undertaken to determine if patient meets criteria for outreach. Patients will be contacted and appropriate appointments made, lab or imaging studies ordered to facilitate the care of the patient. Contact with patient’s primary care provider and staff will be required to ensure coordination of care. The Patient Care Advocate position will coordinate, assist, and implement quality improvement initiatives within Physician Enterprise. This may involve researching data, developing project plans, monitoring quality improvement activities, individual case review, serving as a resource and providing education to team members, providers, and practitioners. The clinical Patient Care Advocate will be involved in educating clinic staff and providers on Quality Measures and processes for satisfying the measures and provide support for ongoing development and maintenance of standards for performance and clinical outcomes.This role will help coordinate and implement a clinical competency program for all “non-provider” clinical staff in the ambulatory clinic settings.

·Facilitates end-user training programs and ensures proper training material is available and provided prior to and during training.
·Provides medical literature research service to the patient, family or health care provider.
·Engage with payors to negotiate a mutually beneficial solution for the patient.
·Communicates effectively and courteously on the telephone and in person with patients and colleagues. Incorporates excellent verbal, written and listening communication skills; and critical analysis skills.
·Fulfills responsibilities such as: contacting patients to schedule Doctors appointments related to quality measures.
·Possess a strong attention to detail, a personal commitment to accuracy and excellence, excellent communication and interpersonal skills and demonstrated organizational skills.
·Designs/implements gap closure optimization programs or processes.
·Prioritize workload and meet deadlines.
·Flexibility depending upon department and/or physician schedule needs.
·Proficiency in applicable EHR systems.
·Understanding of CMS and Payor initiatives and programs.
·Involved in creating education programs which reflect national standards for allied health professionals in an ambulatory/medical practice setting.
·Uses sound education practices to provide clinical and classroom education.
·Assists in development, review and implementation of processes and standards for clinical competencies.
·Assists in developing a variety of training classes to meet specific needs.
·Assists in developing and/or facilitates efficient evidence based, best practice workflows.
·Assists in creating and updating in-service, orientation and continuing education programs.
·Performs other duties as assigned.
·Assists with education and orientation of onboarding clinic personnel.
·Assists with Environment of Care Rounding, provides education and follow up on identified opportunities for improvement to ensure compliance.
Qualifications
The expertise and experiences you’ll need to succeed:
Minimum qualifications:
·LPN – Must have an active LPN in the state of Georgia
·Three years of clinical experience required.
·Three or more years of EMR experience required
·Able to communicate effectively in English, both verbally and in writing required.
·Knowledge of computer systems and applications
·Knowledge of medical terminology
·Working with Word and Excel
