Responsible Under the supervision of a MD or Practice Clinical Supervisor, you will provide care to patients in an ambulatory setting
Principal Accountabilities:
Clinical Functions: Adheres to infection control and hand hygiene guidelines. Recognizes and adheres to National Patient Safety Goals and AMG safety policies. Reports safety concerns promptly via Origami system. Contributes to team and practice site operations by covering co-workers and providers during training, meetings, and lunch and/or as needed. Triage patient calls for emergent issues and medical questions through assessment and collaboration with the interdisciplinary team. Evaluates patient's response to plan of care and collaborates with physician on needed adjustments. Documents nursing assessment and interventions in EMR. Identify learning needs of patients and families, educating by adapting to their learning style and utilizing teach-back method. Provides care that is safe, ethical, patient-family centered, culturally congruent and evidence-based. Skills which may be required to be performed include but not limited to:
Triage via telephone, my-chart or in person
Review and act on test results, initiating initial and follow up consultation and referrals as appropriate
Initiate and monitor allergy skin testing
Allergy/Biologic Injections
Assist with medication management and education
Work independently within the scope of practice to achieve clinical outcomes
Administrative: Documents all communications with patients per EPIC standards. Collaborates with team, checks and responds to email and pools daily. Assists physicians with filling out forms (i.e. disability, worker's compensation, etc). Reviews faxes and responds to requests for information from insurance companies, pharmacies and medical practices. Medication Prior Authorizations and Biologic ordering. Medication and supplies monitoring and inventory. Scheduling appointments as needed.
Required: 1. Graduate of an accredited school of practical nursing required 2. LPN license and registration required 3. BLS certification required 4. Specialty knowledge, skills, and abilities may be required based on department/specialty group assigned.
Preferred: 1. Prior Allergy experience is prefered. 2. Capable of working in a fast- paced environment while maintaining aseptic technique and providing excellent safe patient care. 3. Strong communicator and advocate for patient safety and AORN standards. & HCAHPS/EPIC knowledge preferred 4. Team oriented and team approach to work task accomplishment and unit functions.
