Are you ready to play a pivotal role in patient health and wellness? Join our team as an Annual Wellness Visit Nurse and collaborate closely with our primary care and population health teams to enhance patient care and prevent avoidable hospital visits.
About the Role:
As an Annual Wellness Visit Nurse, you will:
Work collaboratively with our primary care team and care delivery specialists to identify patients eligible for Annual Wellness Visits.
Independently conduct comprehensive Annual Wellness Visits, focusing on prevention and health education.
Ensure timely completion of visits while discussing crucial health prevention strategies and educating patients on related topics.
Coordinate care referrals and leverage community resources to support patient wellness and reduce hospital readmissions and ER visits

Follows AWV protocols and standing orders including completion of preventive services.
Develops and shares knowledge of AWV eligibility and documentation requirements.
Develops knowledge of Medicare preventative services and clinical guideline recommendations for those services.
Addresses open quality gaps with patients and works collaboratively with patients towards closure of those gaps.
Appropriately documents within the electronic record to satisfy closed quality gaps.
Works collaboratively with clinical teams to accomplish AWV in an efficient manner for patient and team. This will include scheduling of the patients and filling open appointments to maintain AWV goals.
Identify the need for patient education. Advise patients on their treatment plan.
Refers appropriate patients to care coordination for improved outcomes
Maintain treatment records/files for each patient
Completes patient encounter form at the conclusion of each service performed
Maintain patient confidentiality; comply with HIPAA and compliance guidelines
Reports outstanding findings to provider immediately
All other duties as assigned within the scope and range of job responsibilities.

Ability to work independently and exercise clinical judgment in interactions with Physicians, payors, patients, and their families.
Strong organizational and time management skills, as evidenced by a capacity to prioritize multiple tasks and role components.
Aware of scope of practice boundaries, comfortable seeking direction and assistance from appropriate resources.
Communication: Ability to communicate effectively verbally and in writing. Excellent negotiation skills.
Computer Skills: Proficient ability to use a computer and electronic medical record.
Confidentiality: Maintain patient, team member and employer confidentiality. Comply withall HIPAA regulations.

Qualifications
Experience & Education Required
Required: Two (2) or more years clinical nursing experience with 1 year in an outpatient setting. Experience with Medicare Annual Wellness Visits as well EHR experience.
Education: Bachelors degree
